CMS 1500 Medicare Forms
Compliance Dates

October 1, 2006:
Health plans, clearinghouses and
other information support vendors must be ready
to handle and accept the revised (08/05)
CMS 1500 Claim Form.
This has changed: Medicare By January 1, 2007.

This is for Health Plans and Clearing houses only. Physicians can still use the old HCFA Forms.

October 1, 2006 - March 31, 2007
Providers can use either the current HCFA (12/90) version or the
revised CMS version (08/05) version of the 1500 Claim Form
Check with your carrier.

April 1 , 2007
The current (12/90) version of the 1500 HCFA Claim form is discontinued;
only the revised CMS 1500 claim form (08/05) can be used from this date forward,
even though earlier submissions may have used the (12/90) HCFA 1500 Claim Form.

May 23 , 2007
Between April 1, 2007 and May 23rd, 2007 all submitted claims (CMS) must include
your new NPI number and all of the other numbers (pin etc.) you currently use on your present HCFA form.
After May 23rd, 2007 Medicare will accept CMS forms with only the NPI number. (See Below]*

No NPI number and or other ID numbers on your form could lead to rejection.
Put old numbers in 33 b with its qualifier.
*The Medicare software is still being developed as of this date (January 10, 2007)

UB04
Compliance Dates

March 1, 2007 - May 22, 2007
Providers can use either the UB04 or the UB92 claim forms.

May 23, 2007
The UB92 is discontinued; only the UB04 claim forms should be used.

Providers should contact their Health plans and Clearing Houses
prior to submitting the revised form to ensure that they are prepared to accept this new form.

All dates subject to change.

Purchase only the correct amount of the old version (12/90) HCFA forms and UB92 forms you need
until you convert to the new version.
Old HCFA 1500 Medicare AND UB92 claim forms are not returnable.
