1500 Medicare Forms Compliance Dates |

1, 2006: Health plans, clearinghouses and other information support vendors must be ready to handle and accept the revised (08/05) CMS 1500 Claim Form. This has changed: Medicare By January 1, 2007. |

This
is for Health Plans and Clearing houses only. Physicians can still use
the old HCFA Forms. |

October
1, 2006 - March 31, 2007 Providers can use either the current HCFA (12/90) version or the revised CMS version (08/05) version of the 1500 Claim Form Check with your carrier. |

May 23 , 2007 No NPI number and
or other ID numbers on your form could lead to rejection. |

only the correct amount of the old version (12/90) HCFA forms and UB92
forms you need until you convert to the new version. Old HCFA 1500 Medicare AND UB92 claim forms are not returnable. |
